RN - Case Manager
Job Description | Provides and directs the provision of home nursing care through competent application of the nursing process and based on rules and regulations set forth to govern such agencies. Coordinates the total plan of care and maintains continuity of care by communicating with other health professionals assigned to the same patient(s). Adheres to the Agency's commitment to providing compassionate and competent care in order to strengthen the health and wholeness of all served.
