Provides skilled nursing care to patients as directed by the CBA Registered Nurse and physician's care plan in accordance with Agency policies. Promotes and maintains patient health through teaching and appropriate rehabilitative measures under the guidance of the CBA Registered Nurse. Complies with all policies and procedures of the Agency and all rules and regulations set forth to govern such agencies.

Provides skilled physical therapy in accordance with the physician's plan of care. Treats patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care. Evaluates, and regularly reevaluates, the rehabilitation needs of the patient; initiates, develops, implements, and makes necessary revisions to the patient's plan of care.
