Community Based Alternatives (CBA) Registered NurseMore DetailsProvides services requiring substantial and specialized nursing skill, in accordance with CBA standards and the patient's plan of care. Makes the initial assessment visit to the patient. Evaluates and regularly reevaluates, the CBA needs of the patient. Initiates, develops, implements, and makes revisions to the patient's individualized service plan. Communicates all changes to designated Texas Department of Aging and Disability Services (DADS) personnel, Agency personnel as well as the patients and/or significant others. Assesses the patient's continual care needs. Addresses all problems in the service plan or documents rationale for not doing so. Assesses and/or initiates diagnostic, preventive, and rehabilitative nursing procedures as appropriate to the patient's care, welfare, and safety. Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, upon changes in the patient's condition, or as mandated. Community Outreach RepresentativeMore Details Acts as advocate among patients, caregivers, Agency personnel, and community members. Directly and indirectly promotes the Agency's services through involvement of planning, implementing, and participating in public awareness activities and events. Represents the Agency in applicable community organizations in order to inform them of the latest information regarding the Agency's services and Federal and State rules and regulations set forth to govern such agencies. Data Entry Clerk - Level 1More DetailsEnters medical information, including patient certification, recertification, post-hospital, and other pertinent data by using Agency software programs. Ensures that all necessary data, forms and documents are submitted to assure timely billing. Prepares admission information, including the Plan of Care, for a patient's medical record. Submits OASIS data electronically and logs submission results. Updates and maintains applicable information such as data entry, new admission, referral logs, admission numbers, ICD-9 codes, and recertification calendar. Serves as a back-up and/or assists in the following departments/areas: billing, medical records, reception, and medical and office supplies. Performs other duties as assigned.
